TEMPORARY ID CARDS

Members who have not received ID cards by January 1, 2012 can follow the instructions below to obtain a temporary ID card.

MEDICAL

CVS Caremark

If a member has not received an ID card by January 1, 2012 and needs to fill a prescription, the member can give the pharmacy the following information to process the claim.

Health Net HMO

Members can go to the Health Net website to download and print an electronic copy of their ID card.

Health Net Seniority Plus

Members can go to the Health Net website to download and print an electronic copy of their ID card.

Kaiser HMO

Members who have not received an ID card and are in need of care can contact the Member Services Call Center at 1-800-464-4000 to obtain their Medical Record Number (MRN) and facility information.  You can then schedule an appointment by contacting the local Appointment center.  At the time of services, provide your California photo ID and MRN.

Kaiser Senior Advantage

Members who have not received an ID card and are in need of care can contact the Member Services Call Center at 1-800-464-4000 to obtain their Medical Record Number (MRN) and facility information.  You can then schedule an appointment by contacting the local Appointment center.  At the time of services, provide your California photo ID and MRN.

UnitedHealthcare® Group Medicare Advantage HMO (formerly known as SecureHorizons)

New members who have not received an ID card should call Customer Service at  800-457-8506 to get their ID number.  You can then register at  www.uhcretiree.com and print a temporary ID card.

 

DENTAL

MetLife affiliated Dental Health Care Service Plan (SafeGuard DHMO)

Members have several options to obtain a temporary ID card.  You can:

-Contact MetLife at 866-576-9121, option 3 to request a new or replacement card.

-Download a temporary card from the MyBenefits website once you’ve registered.

-Click here to download a generic ID card.

                If you do not have an ID card, the dentist can use your SSN to verify eligibility with MetLife.

MetLife Preferred Dentist Program (PPO)

PPO members do not need ID cards.  You can provide your SSN to your dentist to verify eligibility with MetLife.

Western Dental DHMO Centers Only

If you are missing your ID card at the dentist, you can contact the Customer Services Department at 866-901-4416 to verify eligibility.  Your dentist should be able to confirm your eligibility with your CA Driver’s License or CA State ID card.

Western Dental DHMO Plan Plus

VISION

EyeMed Vision Care

Members do not need an ID card when visiting their provider.  Providers can use your name and date of birth to verify eligibility.
